Individual and Family Support Funds

Individual and Family Support Funds are a state funding source from the Department of Behavioral Health and Developmental Services that assist individuals with intellectual or developmental disabilities on the waitlist for Medicaid Waiver home and community based services and their families. Services and items by this program are intended to support the continued residence of an individual in their own or the family home in the community. Funds must be used to purchase specific items or services that will provide emergency supports, a safe living environment, improved health outcomes or community integration. Assistance with moving (including packing, loading/unloading and transporting furniture, household supplies, etc.) is considered community integration. The maximum funding request is $1,000. Applications can be obtained from the DBHDS website and are accepted on a first come, first served basis while funding is available.
